The website uses Google Forms, provided through LifeHealingHeart’s Google Workspace account.

The form is embedded on the Contact page. When you submit a message, your responses are transmitted directly to Google and are not stored in the LifeHealingHeart WordPress website or by its hosting provider, Hostinger.

Submitted responses are stored in the practice’s Google Workspace account. Access to those responses is restricted to the practice owner.

Google Forms does not require visitors to sign in to a Google account, and email addresses are not collected automatically. If you provide your email address in the form, it is used to respond to your inquiry.

LifeHealingHeart receives a notification through its Google Workspace email account when a new response is submitted.

LifeHealingHeart has entered into Google’s HIPAA Business Associate Amendment for applicable Google Workspace services.

Google’s handling of information is also subject to its applicable privacy and security terms.

4. Information Collected Automatically

When you visit the website, certain technical information may be collected automatically by the website hosting provider.

This may include:

  • Internet Protocol (IP) address
  • Browser and device information
  • Pages requested or visited
  • Dates and times of website access
  • Technical information necessary for website operation and security

The website is hosted by Hostinger, which maintains standard server logs.

These logs are used for website operation, security, troubleshooting, and related technical purposes. Contact-form message contents are not stored in Hostinger’s website logs as part of the configured form submission process.

The website’s fonts are hosted locally rather than loaded through Google Fonts.

Hushmail: Provides secure email services used by LifeHealingHeart. LifeHealingHeart maintains a Business Associate Agreement with Hushmail.

LifeHealingHeart does not sell or rent website visitors’ personal information to third parties.

Information may also be disclosed when required by applicable law or when otherwise legally permitted.

8. Information Retention

Contact-form submissions are generally retained for up to 90 days after receipt and then deleted, unless a longer retention period is necessary for treatment, legal, or administrative purposes.

If information from an inquiry becomes part of an established client’s clinical record, that information is maintained separately in accordance with applicable clinical record-retention requirements.

Website server logs and other technical information may be retained by service providers according to their applicable operational and retention practices.

LifeHealingHeart takes reasonable steps to protect personal information collected through its website.

The website uses HTTPS to protect information transmitted between visitors and the website. Contact-form submissions are handled through Google Workspace rather than stored in WordPress.

Access to submitted contact-form responses is restricted to the practice owner.

Visitors are encouraged not to submit detailed medical or other sensitive personal information through the general contact form.

10. Clinical Information and HIPAA

Clinical records and ongoing treatment documentation are maintained separately through Sessions Health, the practice’s electronic health record system.

Sessions Health is not connected to the public website or its contact form.
